Prior to your appointment feel free to download, print, and sign your injection or filler consent form. Just bring it with you to your appointment.  Of course, if you'd prefer, we will provide you one at your appointment.  Additionally, you can download the pre and post treatment documentation or it will be provided at your appointment, as well.
